Request a Fire Marshal Review:
To initiate a request for a Fire Marshal Review:
Complete an application form online by clicking the link below.
You will be required to attach a copy of the Order and all relevant appendices. Acceptable file formats for attachments are .jpg, .png, .gif, .doc, .xls, .pdf, .docx, .xlsx, .tiff, .raw, .jpeg. Your computer must have Chrome (version 64 or higher), Internet Explorer (version 11 or higher), or Firefox (version 58 or higher).
You are encouraged to submit your request for a Fire Marshal Review online. However, if you are unable to do so, you may complete a fillable PDF version of the application form Application for Fire Marshal Review of an Order. Attach a copy of the Order and all relevant appendices to the completed application form. Submit the documents to the Office of the Fire Marshal and Emergency Management using one of the following options:
