Case Submission Forms

The Centre of Forensic Sciences has introduced an electronic case submission form that incorporates “2-dimensional” (2D) barcode technology. This will shorten wait times for “in person” submitters and improve the accuracy of information transfer since all case related data will be seamlessly transferred directly into the Laboratory Information Management System (LIMS).

Calculation of a Blood Alcohol Concentration (BAC) or an opinion on impairment associated with a BAC can be obtained by submitting a 2012-2013 Request for Letter of Opinion form (PDF, 386 kb)
